2019-2020 Private Applicator Training Clinic Registration Form
Notice to all Private Pesticide Applicator License Applicants:
Please note that there are two event types being offered again for the 2020 private pesticide applicator licensing year: 1) training & testing clinics; and 2) testing-only sessions. Pre-registration is required for all clinics and sessions. If you pre-register for a clinic and do not attend or do not cancel prior to the event, you will be charged the registration fee. For confirmation of registration, please send a self-addressed stamped envelope. Check-in at training & testing clinics begins at 7:30 AM. Start times at testing-only sessions vary so check details for the specific session of interest. Training & testing clinics, as well as, testing-only sessions, will normally be held regardless of the weather. For any changes, call or visit the website at www.pesticidesafety.illinois.edu.
There remain four ways to obtain a private pesticide applicator license:
1. Attend a combination Training & Testing Clinic and successfully complete the state examination;
2. Attend a Testing-only session and successfully complete the state examination;
3. Participate in•the On-line Self..Study training program and then successfully complete the state examination at any clinic or at the Department's office (by appointment) in Springfield or DeKalb; or
4. Successfully complete the state examination at the Department's office (by appointment) in Springfield (217/785-2427) or DeKalb (815/787-5476).
Training Fee for Training & Testing Clinics: $40 per session per attendee
Please note that the training fee is per training clinic per attendee and does not include the
license fee. If you successfully complete the examination, the Illinois Department of Agriculture
wiJI forward to you an application form which must be completed and returned to the Department
along with the applicable license fee. There is no fee to take the examination.

Testing-only Sessions:
To register, please call the listed telephone number associated with the Testing-only Session site of interest. Please use online registration if listed for your test-only location.
Self-study training via the internet training option:
There are two on-line training programs available for private pesticide applicators: PrivateApplicator Training ($15 fee); and Grain Fumigation Training ($15 fee). Users will have access to the training materials for one year from the date of purchase. This is for on-line training only. The on-line training link is as follows:www.pesticidesafety.illinois.edu
Please be advised that state license examinations are not offered via the internet and must be
taken at scheduled testing locations. Grain Fumigation examinations will be available at all
Training & Testing clinics, Testing-only sessions, and by appointment at the IDA's Springfield (217/785-2427) and DeKalb (815/787-5476) offices.
