  • Providing nutrition education classes for patients
  • Providing trainings for staff on topics such as food insecurity screens and local food access resources
  • Training, project needs assessment and action planning support for establishing food insecurity screening and referral program
  • Assisting with Community Health Needs Assessments, Community Health Improvement Plans, IPLANs, community coalition collaborations, etc.
  • Planning, providing technical assistance, training and grant write support to start a food pantry or Fruit & Veggie or Play RX Program at your facility
  • Connecting with farmers or community partners to host a Farmers Market that accepts Link Up IL, WIC Farmers Market Vouchers or  Link Double Up Bucks
  • Establishing a donation garden with planning, training and funding support
