2019-2020 Group Enrollment Forms
This training is required for all NEW participants wishing to introduce Embryology into their classrooms, or those who need the full training module. Content will be oriented to train teachers or homeschool leaders to conduct classroom embryology.
Beginner / Refresher Training is Wednesday, February 26, 2020, at - Lake County Extension office, 4:30 - 7:30 p.m. During training, you will have the option to participate in the embryo dissecting process. Please indicate if you will take part in this when registering below.
Cost of Beginner/Refresher Training - $50 (includes training material, 2 dz. fertilized eggs, & chick feed)
TO REGISTER ONLINE (complete all steps shown below) - paying by credit card, cash or check... by February 21, 2020.
The following MUST BE RECEIVED at Extension Office by Feb. 21, 2020:
IL 4-H Group Enrollment Short FORM - white form mailed to previous Embryology participants; (Any teacher or homeschool parent that teaches Embryology through Lake County Extension is REQUIRED to complete and return this document to Lake Co Extension office. Complete and indicate Advanced or Beginner Embryology.

FULL payment - Credit card payment option available online; also cash or check payments option available online and then mail-in by February 21, 2020. Make checks payable to the 'University of IL Extension'.

INCUBATOR: You may purchase your incubator from Grayslake Feed; 21 N Seymour, Grayslake, 847-223-4855.
OR receive a loaner from Extension for $20. Call Sabrina at 847-223-8627 to reserve one ASAP; limited quantities.
Incubators must be up and running at least one week before egg pick up day, by March 31, 2020. Egg pick-up is Tuesday, April 7, 2020, 6:30 am - 1:00 pm, at the Extension Office.
CONTACT INFORMATION should consist of the participant's name (ex. teacher or homeschool leader's name), and the SCHOOL's address. Each participant (or one classroom) should register separately.
