Illinois 4-H Project Guide 2021-2022

Re-enrollment information for 2021-2022 was emailed to all current members.  New families should get connected with a local club, and your club leader will have enrollment information for you.

NEW! 4-H Project Process Booklet - This will guide you through the process of how to do a 4-H project.  Print a copy for each project you are doing and fill it in throughout the year.

Project Book Request Form - Members in Logan, Menard, and Sangamon counties can receive up to 3 projects books each year.  Request books you need by using this online form.

Illinois 4-H Webstore - Members who need more than 3 books in a year can purchase those additional books here.

Assumption of Risk Form - Must be signed by each 4-H member and parent one time during 4-H career and submitted to 4-H office.

4-H Program Fee Waiver Request Form - If the annual program fee ($20 per member) is a financial hardship to your family, print and complete this form and return to 4-H office.


Club Activities

4-H Club Organizational Sheet - After reorganizing for the year and electing officers, leaders should complete this form and turn it in to county staff.

Sign-in Sheets - For in-person club activities, ALL attendees (including family members and guests) should sign in, and the leader should keep the list for at least 30 days.

4-H Club Fundraiser Form - Must be completed and submitted to 4-H office two weeks before planned fundraiser.

4-H Club Secretary and Treasurer Books - Link to Illinois 4-H Club Resources webpage.

4-H Club Reporter's Form - Should be completed and submitted to county staff by the Reporter after each club meeting.

Attendance Report (Logan County only) - Should be filled out at each meeting and submitted to Carissa Davis, akpore@illinois.edu.
