Online Needs Assessment Survey

We are conducting a Community Needs Assessment of our 5 county area that we serve, through an online survey. The purpose of the needs assessment is to learn more about the issues facing our communities in our Unit (Edwards, Lawrence, Richland, Wabash and Wayne counties). The information will be used to help us plan and design educational programs to address the emerging needs of our communities.  Individuals can take part in the survey by clicking on the link to the survey (which I've listed below). The survey will be open April 1-30, and takes just a few minutes to complete either online or via a mobile device. As an incentive, the names of those who complete the survey and list their contact information will be put into a drawing for a $50 fuel gift card. Gift cards have been donated by Doug Raber - State Farm, Lawrence County Memorial Hospital, and Terry Bruce- Mt. Carmel Public Utility.

This survey is open to the public.  Everyone in Edwards, Lawrence, Richland, Wabash and Wayne counties are encourages to take the survey.
