Strengthen your health with practical tools for managing chronic conditions.

Learn to thrive, not just survive, with chronic conditions such as arthritis, diabetes, and hypertension. Explore how to make informed choices about nutrition and exercise, and learn about available treatment options. Discover strategies for managing pain, communicating effectively with health care providers and loved ones, and using tools to support long-term disease management.

Event details

The webinars will be held weekly for six weeks in 2.5-hour sessions from 9 to 11:30 a.m., with a built-in break. Session dates are April 29; May 6, 13, 20, and 27; and June 3 and 10.

Topics include, but are not limited to:

  • Appropriate exercise to maintain and improve strength and endurance
  • Fall prevention
  • Healthy eating
  • Better breathing techniques
  • Proper use of medication
  • Working more effectively with health care providers
  • Communicating with friends and family
  • Communicating with oneself
  • Communicating within the health care system
  • Action planning, problem-solving, and decision-making

Attendance:
This evidence-based program requires full attendance and is intended for adults aged 18 and older living with chronic conditions.
