Community Health Team
The mission of the Community Health Team is to promote inclusive activities for Cook County residents by sharing practical health information and resources to enhance skills that can be applied to support health and well-being.
The team's vision is to educate, prevent, and inspire and works to:
- Educate individuals about risk factors for various health conditions
- Prevent or reduce the chances of negative health outcomes among individuals
- Inspire behavior change among individuals that will eventually impact the health of their communities
University of Illinois Extension offers a variety of health programming in Cook County. These sessions inform participants about health topics that can affect them and their families. Extension educators reach communities in-person and virtual programming as well as through their websites and blogs. Through Community Health: Education, Prevention & Inspiration, educators impart practical information you can trust.
